PRIVACY STATEMENT

In accordance with the Privacy Act 2020

When you book an appointment at The Synergy Centre, or if you subscribe to our mailing list - either online, at one of our events or at an event we attend - your name, email address and contact number is added to our client database.  We only use these details to contact you about your appointment/s or to send you our newsletter or updates. Should you wish to unsubscribe from the newsletter, there is an unsubscribe option at the bottom of each one.

We keep your contact information, information provided in the client intake form and any other information documented during consultation or at your appointments safe by storing it in a secure electronic file.  We do not obtain any information about you without your knowledge and consent and we strictly do not share your information with anyone else, unless required by law or if you specifically request us to in writing.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you’d like to ask for a copy of your information, or to have it corrected, please contact us at wellbeing@thesynergycentre.co.nz

We keep your information for 2 years from the date of your last appointment or interaction with The Synergy Centre, at which point we securely destroy it by securely erasing/deleting it.

The Synergy Centre Terms & Conditions

  1. To secure any future appointments, payment must be made in advance. 

  2. Please ensure you arrive for each appointment on time as lateness of more than 15 minutes may cause your appointment to be cancelled. (See point 3.)

  3. If you are unable to attend any appointments, please provide at least 24 hours notice.  If you provide at least 24 hours notice your appointment can be re-booked or your payment refunded.  If you do not provide at least 24 hours notice the payment may not be refunded.

  4. Contact between appointments will be limited to telephone or email.  Please not turn up at the centre unannounced.

  5. You must provide an email address and contact telephone number that you can be contacted on at short notice in the event of any unforeseen circumstances.  

  6. Anti Social behaviour – including verbal abuse, will not be tolerated and will result in immediate cessation of the session and any course of sessions.

  7. This consultation form and any notes, personal or sensitive information taken during consultation or any following sessions will be held securely and in accordance with the Privacy Act 2020. 

  8. The Synergy Centre will not obtain any personal/sensitive information about you without your knowledge or consent.

  9. All contents of your personal file will be available to you upon written request. 

  10. No information will be disclosed to any third party without your explicit consent.  If you do wish any of your personal details or information discussed with you to be passed on to any other third party this must be requested by you in writing stating the name and address of the person/entity you are requesting the information be disclosed to.

  11. The strictest of confidentiality will be maintained in all but the most exceptional circumstances. Exceptional circumstances can only include: legal action (criminal or civil court cases where a court order is made demanding disclosure – includes coroners courts) and where there is good cause to believe that not to disclose would cause danger or serious harm to yourself or others. Most standards of confidentiality applied in professional contexts are based on the Common Law concept of confidentiality where the duty to keep confidence is measured against the concept of ''greater good”.

  12. The sharing of anonymous case histories with peers and peer-support groups is not a breach of professional confidentiality. The sharing of open case histories with peers or any referring medical practitioner is also not a breach.

  13. Any CD’s or MP3’s you may be given following a session are copyright and for your use only.  They must not be copied or given to others to use.  Any unauthorised copying or use of the CD’s/MP3’s would result in breach of copyright and breach of these terms and conditions.

  14. You understand that Shannon Douglas or any representatives of The Synergy Centre does not claim to diagnose, treat or cure any condition, illness or disease or to replace the medical advice of your doctor or health care professional.  Any treatment, therapy or information you receive is not intended as ‘medical’ advice or treatment and may not be used as such.  You are advised to consult with your doctor or specialist health care professional with regards to matters relating to any health condition, and any recommended supplements if you are pregnant or taking medication.

  15. You agree to release, indemnify and hold harmless Shannon Douglas, The Synergy Centre and any representatives of The Synergy Centre for any consequences that may arise from your own thoughts, decisions and actions, or any loss or damage you may suffer in any way, associated with any treatment, therapy or participation at The Synergy Centre.
